 This application form should be completed IN FULL by applicants from very needy families who attended a public primary and Junior school, and have been admitted to a CLUSTER 1 SCHOOL or have scored a Pathway Score of at least 60 and are admitted to a CLUSTER 2 Secondary School. Students from private primary schools DO NOT QUALIFY.

This application form MUST be filled out by the applicant unless otherwise specified.
